First Visit

During your first visit, your physical therapist will take a detailed history and assessment of your injury, get to know you and establish a game plan to return you to a healthy lifestyle. 

This initial evaluation includes necessary tests for strength, balance, posture, gait, coordination, flexibility and pain in order to determine all factors contributing to the symptoms that brought you to us in the first place.  The results of this examination, coupled with a review of your functional limitations and personal goals, will help us to design a customized treatment plan specific to your needs.   

The primary goal of the initial appointment with us is to fully understand your clinical picture and to establish a mutually agreed upon treatment plan.  Depending on the complexity of your presentation, this may take the entire session or only a portion of the session.  Any remaining time in your session will be utilized for initiating treatment and educating you in ways that you can begin to contribute to your recovery at home.   Please plan to be here for 60-75 minutes for this first session.
After your first visit, you will schedule follow up appointments with our front desk team.  Please allow 45-60 minutes for each follow-up physical therapy session.  You will be scheduled for follow-up visits with the same physical therapist that evaluated you.  If your schedule is not compatible with this PT, we will accommodate you in another therapist’s schedule.  Our therapists practice similar philosophies of treatment and meet weekly to discuss any shared patients.  While we pride ourselves in a team approach, it is best for consistency of care that you schedule with no more than 2 therapists.
The frequency and duration of your treatment depends upon your condition, your activity level and your participation in your home program.  We will work with you to determine an appointment schedule that meets your needs.

Please bring the following with you to your first appointment:

• Completed registration forms: If you cannot download these in advance, please plan to arrive 20 minutes early to complete your paperwork. 
• A prescription and/or post-surgical protocol from your doctor if you have one
• Any reports from tests related to your injury (MRI, CT scan, etc)
• Your insurance card for all non-workers’ compensation patients
• Your C2 form for all workers’ compensation patients
• Clothing that allows you to move easily and allows access to your injury.  Tank tops for upper body and shorts for lower body injuries work best.  We have changing rooms and day use lockers for your convenience.  If you forget, we have clothes you can borrow.
